We would like to inform you that each company have to resubmit the following documents from 1st January 2012 before applying Certificate of Origin:
1. Form 9 and Form 13 (Certificate of Incorporation of a Company), Form D and Form B (Certificate of Registration of a Business)
2. Director’s Statutory Declaration together with Form 49
3. Specimen of authorized signatories together with Photostat copies IC/passport
4. Manufacturing License / Other licenses (eg. MPK License)
5. Letter of Indemnity
